Job Description Job Title : Transition Coordinator (JJD / Youth Services) Location : On-site – Indiana (Juvenile Correctional Facility) Work Schedule : Monday – Friday Job Summary The Transition Coordinator supports at-risk youth in juvenile correctional facilities by facilitating life skills programs, coordinating reentry planning, and connecting students with education, employment, and community resources. This role works closely with teachers, state agencies, and workforce partners to ensure successful transition back into society. Key Responsibilities Facilitate life skills curriculum ( Next Steps ) for youth in the facility. Support and evaluate student Transition Consultations and reentry plans. Participate in Administrative Release Committees (ARC) to review student progress and readiness. Conduct follow-up calls with released youth at 1, 3, 6, 9, and 12 months . Collaborate with agencies such as Department of Workforce Development, HIRE, and mentoring programs. Assist in developing Individualized Education & Learning Plans . Coordinate with state, county, and private organizations to provide reentry services. Help students obtain essential documents (ID, Social Security card, driver’s license, etc.). Support transition back to schools, employment, apprenticeships, or military pathways . Represent the organization at meetings, conferences, and community events . Required Skills & Qualifications : Strong ability to communicate effectively in a team environment Experience training, coaching, or mentoring youth Experience in collaboration and program/initiative development Highly Desired Skills Experience as a spokesperson or presenter at events Experience building partnerships and community relationships Work Environment On-site role in a juvenile correctional facility May require supervising students in a classroom setting Must comply with all background checks (drug screen, TB test, fingerprinting, etc.) Must be available for full-time on-site work (Mon–Fri) Must accept $21/hour pay rate Must adhere to attendance and leave policies Ideal Candidate Passionate about working with at-risk youth Strong interpersonal and communication skills Ability to work in a structured, secure environment Committed to helping youth successfully reintegrate into society
