Job Description:

Elite Home Rehab and Healthcare is an accredited ACHC skilled home health agency servicing Central Indiana. We are a clinician owned agency, dedicated to providing quality care to our patients as well as helping our clinicians grow professionally. We are looking for a part time speech therapist for the greater Indianapolis, Indiana area. We are currently offering a sign on bonus of $500 for part time employment.

Duties of Position:

Provide skilled speech language pathology service on an intermittent basis to patients in their home in accordance with the physician's orders.

Position Responsibilities:
Provided initial and ongoing comprehensive assessments of the patient's needs in including Outcome and Assessment Information Set (OASIS) assessments.
Establish a Speech Therapy treatment plan in consultation with the physician and revises it with the approval of the physician as necessary.
Assist in development and implementation of the interdisciplinary patient care plan as it pertains to Speech Therapy.
Teach, supervise and counsel the family and patient in the total Speech Therapy program and other related problems of the patient at home.
Record on a timely basis all evaluation data, treatments, and patient's reaction to treatment and/or any changes in the patient's condition.
Communicate with the physician at least every 30 days or whenever changes occur regarding the patient's status and obtains additional orders, if necessary.
Participate in agency activities, case conferences, and meetings, when appropriate.
Participate with staff, patients, and physicians in discharge planning activities and completes a Speech Therapy Discharge Summary.

Qualifications:
Must have the ability to drive and access patients' homes.
A Master's Degree in Speech-Language Pathology with a Bachelor Degree in the Field of Speech and Hearing Therapy preferred.
Licensed in the State of Indiana to practice speech language pathology.
Preferred to have at least one year of institutional experience.
Must have a criminal background check.
Must have a current CPR certification.

Job Types: Part-time, PRN

Pay: $60.00 - $70.00 per hour
