works for you. What You’ll Do Provide in-home respiratory
assessments, treatment, and follow-up care for patients with COPD,
asthma, CHF, ventilator or tracheostomy needs, and other chronic
respiratory conditions. Educate patients and caregivers on
equipment operation, oxygen safety, and preventive strategies.
Partner closely with nurses, physicians, case managers, and family
to coordinate seamless transitions from hospital to home. Monitor
patient progress, report changes promptly, and support
physician-directed Plan of Care/Treatment for optimal outcomes.
Serve as a trusted advocate and motivator — empowering patients to
take control of their health journey. What You’ll Bring Current IL
Respiratory Therapist license (RRT or CRT) and BLS certification.
Previous experience in home health, subacute, or pulmonary rehab is
strongly preferred. The ability to supervise clinical personnel and
have a basic understanding of insurance requirements (Medicare,
Medicaid, and private insurance). Comfortable working independently
while maintaining strong collaboration with the care team. A
genuine passion for meeting patients where they are — literally and
