growing hospice provider is seeking a compassionate, experienced
Hospice Registered Nurse (RN) Case Manager to join its team. This
is a full-time, field-based role offering flexible scheduling,
strong leadership support, and the opportunity to make a lasting
impact on patients and families during their end-of-life journey.

Highlights: Branch Census: ~30 patients Caseload / Average Visits:
Caseload of approximately 12 patients; daily visits vary but
typically average 4–5 per day. Several partner facilities have
multiple patients. Leadership: Works closely with the Executive
Director and Director of Clinical Services Schedule: Monday–Friday
daytime schedule with an on-call rotation. Rotates with RN,
Executive Director, and Director of Clinical Services; includes
some weeknights and at least one weekend per month. Territory:
Preference for candidates living in the southeast area and able to
focus on patients in the Greenwood/Franklin area. Occasional
coverage may be required in other parts of Indianapolis (including
the northeast side such as Carmel, Fishers, and McCordsville).
On-call coverage may include any service area. EMR: Netsmart Key
Responsibilities: Conduct physical and psychosocial assessments and
develop individualized care plans Manage pain and symptoms using
evidence-based interventions Provide education and emotional
support to patients and families Collaborate with interdisciplinary
team members for coordinated care Accurately document all
assessments, care plans, and follow-up in the EHR (Netsmart)
Participate in on-call rotation and quality improvement efforts
Qualifications: Active RN license Hospice or home health experience
preferred but not required Open to candidates with 2 years of
Med-Surg or critical care experience; prior exposure to end-of-life
care or home-based settings is strongly valued Strong assessment,
documentation, and communication skills Committed to providing
