Responsibilities
The Care Coordinator 2 (CC2) provides professional mental health
care and case management of assigned patients under daily
leadership of unit supervisor. Works closely with the psychiatrist
to ensure appropriate treatment and discharge planning for the
patient. The CC2 works closely with other members of the social
services team. We are currently looking for a CC2 for our Youth
Acute population and for our Adult

Qualifications
Education/Licensure: Master's Degree (MSW) from an accredited
college or university, in Social Work or a clinical related mental
health fieldExperience: Previous experience in a psychiatric health
care facility, with direct experience in family, individual and
group therapies, case management, crisis intervention, and
treatment skills, preferred. SBIRT: Understanding of Mental Health
Dx and treatment / Implement Trauma Informed Care.Additional
Requirements: License in provision of behavioral health clinical
treatment preferred (LSW.)Additional Information: Required to work
