currently seeking a Behavioral Health Consultant (BHC) to join our
Arlington Health Center. This position is ideal for a licensed
therapist who is passionate about integrated care, enjoys
fast-paced primary care or pediatric settings, and thrives in a
collaborative, team-based model. Job Summary The Behavioral Health
Consultant (BHC) works alongside Primary Care Providers and
Specialty Medicine Providers, serving as an integral member of the
medical team to address a broad spectrum of behavioral health needs
within the clinic population. The BHC conducts brief
biopsychosocial assessments, provides solution-focused,
evidence-based interventions, and supports early identification,
treatment, and prevention for as many patients as possible. The BHC
provides same-day consultative services, assists in managing
chronic conditions, supports psychoeducation, and works closely
with the care team to improve recognition, treatment, and
management of psychosocial and medical conditions. The BHC also
provides brief follow-up, triage, and referral to specialty mental
health care when needed. Essential Duties & Responsibilities
(include but are not limited to): Patient Care & Intervention
Provide behavioral health assessments, diagnostic impressions, and
brief psychotherapeutic interventions. Deliver same-day brief
interventions for coping strategies, treatment compliance, and
behavioral change. Conduct risk assessments and support crisis
intervention. Provide brief follow-up, relapse prevention, and
education. Support treatment of chronic conditions (e.g., diabetes,
insomnia, pain, medication adherence). Proactive Identification &
Care Coordination Review/scrub provider schedules daily to identify
patients with unmet behavioral health needs. Proactively recommend
same-day BHC consultations to medical teams. Participate in daily
huddles, rounding, and interdisciplinary team meetings. Collaborate
with enabling services and referral departments for case management
needs. Triaging and referring patients to specialty behavioral
health when appropriate. Offer ongoing consultation and monitoring
for patients who require continuity services. Integrated Team
Collaboration Provide timely and succinct feedback to medical
providers with consultation findings and recommendations. Assist
physicians, advanced practice providers, and medical staff in
managing overall patient health. Work as a core member of the care
team to build clinical pathways or best practice programs for
targeted populations (e.g., chronic pain, diabetes, heart disease,
obesity). Participate in team-based care activities and integrated
treatment planning. Deliver services in exam rooms when possible to
reduce stigma and reinforce team-based care. Education & Program
Development Provide psychoeducation and culturally responsive
behavioral strategies tailored to patient needs. Develop and teach
classes that promote wellness, skill-building, and prevention.
Provide behavioral health education to medical teams on
evidence-based interventions and integrated care best practices.
Share knowledge with team members formally (in-services,
documentation) and informally. Assist in developing BHC response
protocols for key medical conditions and treatment issues.
Documentation & Compliance Document all patient encounters in the
Electronic Health Record (EHR) in a timely and compliant manner.
Ensure documentation aligns with JPCHC, state, county, and federal
reporting requirements. Deliver patient care that is efficient,
accurate, culturally responsive, and customer service-oriented.
Required Skills and Qualifications: Master's Degree in Clinical
Psychology, Counseling Psychology, Social Work, or related
behavioral health field (required) Indiana licensure as LMHC, LMFT,
or LCSW , or license-eligible (required) Training and experience in
pediatric behavioral health (required) Prior integrated or
team-based care experience (strongly preferred) Ph.D. or Psy.D. in
Clinical or Counseling Psychology from an APA-accredited program
(preferred) Knowledge of state and county reporting requirements
Proficiency in Microsoft Office and Electronic Health Records (EHR)
